This application is for Physicians and Practitioners who have been practicing for 24 months or longer. If you are a student or a recent graduate, please use the provisional application.

AMI Credentialing is offered to all licensed CAM providers. If you are an MD, DO, ND, DC, OMD or Licensed Massage therapist or Acupuncturist you are eligible to complete the application for either the PPO, HMO or Integrative Chronic Pain Management Program.

The AMI Integrative Chronic Pain Management (DM) Program, currently in Florida, is Credentialing Licensed Acupuncturists (L.Ac), Chiropractors (DC), Massage Therapists (MT), Nutritional Counselors (RD/LD), and Pharmaceutical Consultants (PharmD’s).

AMI’s products can be divided into three categories:

1. The PPO product is an open access program fully integrated with (MD) oversight and co-management.
2. The HMO product is a ‘gatekeeper’ model where CAM physicians serve as Primary Care Physicians (PCPs). This is a capitated program. Here, too, Medical Doctor (MD) medical directors provide integrative oversight and co-management.
3. The Integrative Chronic Pain Program integrates conventional medical practices with complementary alternative medicine for members with chronic back and neck pain, fibromyalgia, chronic fatigue syndrome, and related disorders. Case Management and coordination of care is be provided by registered nurses specializing in this field.

Because the medical, legal, and contractual responsibilities of CAM providers differ between these models, each has it’s own credentialing criteria and clinical assessment tools. There are separate application forms for each model, so please decide whether you wish to participate in the HMO or PPO or Integrative Chronic Pain Management products and respond below accordingly.
